GETTING STARTED AT REBIRTH: When booking your first appointment with us, you will be prompted to upload your credit card. Do not worry, we will not charge your credit card until after your appointment with us. If you do not have a credit card, please call or email us to book your first appointment.

INTAKE FORMS & CONSENTS: When you book your initial appointment for any service, you will be emailed an intake form and consents immediately after to complete. It’s easy to forget and put it aside, but it contains information that your health practitioner needs and your signed consent for treatment. Intake form MUST be completed at least 24 hours prior to your visit. You will receive an email reminder as well as a phone call from our office admin if it is not completed the day before your appointment.

BOOKING FOR YOUR CHILD: Please create a separate profile in Jane with your child’s name and date of birth. The username needs to be different from your username in Jane, so something like your child’s name will help you remember. It doesn’t need to be an email address. You can connect their profile in Jane to your Parent Profile. If you need help doing this, our admins can easily do this for you on their end. .

DIRECT BILLING: We offer direct billing for most services to most major insurance companies, including Canada Life, Sun Life, Manulife, Greenshield, Blue Cross and more. In order to do this for you, please complete the information in your intake form.

PARKING: For Central Ave location downtown, we have limited parking of 6 spots behind our building along the wooden fence that is labelled “Rebirth Wellness Centre”. The other parking spots are reserved for residents that live there and are marked with a “R” for residents, and you cannot park there. You can get up to 1-hour of FREE PARKING. by using the promo code CORE on the HONK APP.
