We're located at 1240 Commissioners Road West, Unit 206.
When you arrive, look for the wall of glass windows and doors under the Active Physio sign. That is the main entrance to the office building. You can take either the elevator or the stairs up to the second floor. Once you're up there, you'll find Rebirth at the end of the hall on the left.
